What information do I need to report a street light fault?

It would be helpful when reporting a fault if you could supply the following information:-
Appliance Number - All lighting columns, traffic signs, illuminated bollards and zebra crossing poles are numbered.
Location - The name of the street where the appliance is located, and the position of the appliance, i.e. the house number it may be outside, whether it is near a junction, on a footpath etc.
What is the fault - Light out, flashing on/off, burning red or dim, out of time, lit all day / night.
